The following is a step by step document for applying for Financial Aid. You can download the document here.

Financial Assistance Scholarships 

Financial assistance from Tulsa Adventist Academy is awarded to qualifying families who have successfully completed applications with BOTH the Opportunity Scholarship Fund (OSF) and FACTS Grant & Aid. Due to limited resources, financial assistance is first awarded to our constituent church families then to the remaining families as funds allow. 

 

Requirements to receive financial assistance:  

 

  • Students must maintain a GPA of 2.0 or higher 

  • Students must maintain an attendance record with no more than four (4) unexcused absences per quarter 

  • Students must have no behavioral issues throughout the school year 

  • Family must donate a minimum of four (4) hours volunteering for the school 

The failure to meet any of these requirements results in the loss of the scholarship. 

 

 

STEPS TO APPLY 

 

Step 1: Enroll your student at Tulsa Adventist Academy  

Step 2: Apply for the Parental Choice Tax Credit, if eligible  

  • Opens February 17 (priority window through April 19) 

Step 3: Apply for an OSF Scholarship, if eligible  

  • Opens April 15 

Step 4: Apply for Grant & Aid through FACTS  

  • Opens Feb 1 (priority window through July 31) 

  • $45 required application fee  

 

 

FINANCIAL ASSISTANCE APPLICATION INSTRUCTIONS 

 

 

Step 1: Enroll your student at Tulsa Adventist Academy 

 

 

Step 2: Apply for the Parental Choice Tax Credit, if eligible 

 

  • You do not need to create an account.   

  • Scroll to the Parental choice Tax Credit section 

  • Select Parental Choice Tax Credit Taxpayer Application 

  • You will need the following: 

  • Taxpayer(s) and/or parent/legal guardian's information, including name, address, phone number, email, Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN). 

  • Case Number/Member ID and Confirmation of Benefits for recipients of income-based government benefits including Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF), or SoonerCare/SoonerSelect.  

  • Adjusted Gross Income (AGI) of the student's parent or legal guardian during the second preceding tax year. For example, for the school year 2025-2026, the second preceding tax year would be 2023. 

  • Individuals who have not yet filed a 2023 Oklahoma income tax return will need to file their required 2023 Oklahoma tax return prior to applying OR submit a copy of their 2023 IRS Form 1040. 

  • Individuals who did not have an Oklahoma tax filing requirement for 2023 will need to attest to not having a filing requirement. 

  • IRS Form 1040, if applicable 

  • Student information, including name, date of birth, and Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN). 

 

 

Step 3: Apply for an OSF Scholarship, if eligible 

 

Tulsa Adventist Academy partners with the Opportunity Scholarship Fund to raise scholarship funds for our school and issue financial aid. OSF Applications are required for the funds to be released to TAA on your behalf. 

 

How to Qualify for an OSF Scholarship 

 

  • Be a legal U.S. resident living in Oklahoma 

  • Be accepted by an OSF member school (complete enrollment at TAA) 

  • Total annual income for the family during the preceding tax year is less than the amount listed in each bracket below. For example, if you have a family of four (4) and your total annual income was less than $178,433 last year, then you are eligible! 

  • Family of 1: up to $86,858 

  • Family of 2: up to $117,383 

  • Family of 3: up to $147,908 

  • Family of 4: up to $178,433 

  • Family of 5: up to $208,958 

  • Family of 6: up to $239,483 

  • Family of 7: up to $270,008 

  • Family of 8: up to $300,533 

 

Complete the OSF Application 

 

  1. Visit www.osfkids.org/scholarships and click the “Apply Online” button. You will be redirected to an application portal, where you will create an account.  

  2. Create an application for each student applying for a scholarship.  

  3. Locate a copy of your most recent 1040 tax form and proof of enrollment, such as an email screenshot, scanned letter, etc. (Your proof of enrollment letter was emailed to you from Facts).   

  4. Upload documents to each application and submit.  

 

 

 

Step 4: Apply for Grant & Aid through FACTS: $45 non-refundable application fee   

Tulsa Adventist Academy contracts with FACTS to provide financial aid assessment services. Families provide financial information directly to FACTS, which remains strictly confidential. FACTS compiles the information to evaluate the financial need status of each applicant, which in turn is communicated to Tulsa Adventist Academy, where a final determination is made.  

 

To provide your educational institution with an accurate assessment, FACTS Grant & Aid Assessment must verify your expenses and household income. By providing us with all your information, we can assist your institution in making an informed decision about the amount of financial aid needed for each family. 

 

All documentation received is imaged upon receipt and then destroyed. You may login to your FACTS user account to review the status of your application. Please allow two (2) weeks’ processing time before inquiring further about receipt and/or status of the uploaded documents. Application deadlines are set by TAA. If you are applying after the deadline, contact TAA to ensure that your application will be accepted. 

 

  • If you do not have a Social Security Number, you may use a Visa number, a TIN (Tax ID) number, a passport number, or another 9-digit identifier. 

  • NEW this year: FACTS Grant & Aid includes a powerful new IRS Integration tool, allowing families to automatically retrieve tax transcripts from the IRS—no more manual uploads, fewer errors, and faster processing. This means: 
    ✅ Less paperwork for families 
    ✅ Faster verification for schools 
    ✅ A smoother financial aid process for everyone 

  • If you upload tax documents rather than using the integration tool, you will need the following: 

  • IRS Federal Income Tax Return, including all supporting schedules. If applicant and co-applicant file separately, both tax returns are required. 

  • All current year W-2 Wage and Tax Statements for both the applicant and co-applicant.  

  • IMPORTANT: If you file a tax return but do not have W-2 wages because you are self-employed, submit a copy of your current year Federal Form 1040 Tax Return. 

  • NOTE: If you are applying before you have received all the current year W-2 Wage and Tax Statements, please submit them as soon as they become available.

  • All supporting tax documents if you have business income/loss from any of the following:  

  • Business - Schedule C or C-EZ and Form 4562 Depreciation and Amortization  

  • Farm - Schedule F and Form 4562 Depreciation and Amortization  

  • Rental Property - Schedule E (page 1)  

  • S-Corporation - Schedule E (page 2), Form 1120S (5 pages), Schedule K-1 and Form 8825  

  • Partnership - Schedule E (page 2), Form 1065 (5 pages), Schedule K-1 and Form 8825  

  • Estates and Trusts - Schedule E (page 2), Form 1041, Schedule K-1  

  • Copies of all supporting documentation for household Non-Taxable Income such as: Social Security Income, Welfare, Child Support, Food Stamps, Workers’ Compensation, and Temporary Assistance for Needy Families (TANF). If you do not file a tax return, you are required to provide documentation of all income received. 

  • Supplemental Required Documentation: Member in Good Standing Form 

  • This form can be obtained from the school office.   

  • The top of this form is to be filled out by the applicant and then given to your local church pastor to complete. It can then be emailed back to Tulsa Adventist Academy at treasurer@tulsaacademy.org or mailed to Tulsa Adventist Academy, Attn: Treasurer, 900 S. New Haven Avenue, Tulsa, OK  74112.
